Medical Home or Usual Source of Care
Data from the Ohio Family Health Survey (OFHS), released in March, show the percentage of adults who report NOT having a medical home or usual source of care. Ranges are from a high of 25.2% in Hocking County to a low of 5.5% in Putnam County. The percentage of adults who report not having a medical home in the state of Ohio is 14%. People who do not have a medical home are less likely to seek appropriate and timely healthcare when they need it.
Also shown are the current (June 2009) Federally Qualified Health Centers (FQHCs). FQHCs are community-based organizations that provide comprehensive primary care and preventive care, including health, oral, and mental health/substance abuse services to persons of all ages, regardless of their ability to pay.
